Social Care Facilitator

\nRate of pay: £18ph PAYE
\nWorking Pattern: 37 hours
\nWorking Structure: Hybrid - 2 mandatory days in the office
\nDuration: 3 Months
\nEssential requirements: A DBS check may be applicable for this role & Driver
\n\nOpus people solutions are currently recruiting for a Social Care Facilitator for our client based in Walsall on a temporary on-going basis for 3 months.
\n\nAbout the Role
\nYou will work as part of a multi-skilled team responsible for meeting the assessment, care management and support needs of vulnerable adults and their carers.
\nThe role focuses on enabling individuals to exercise choice, maintain independence and access appropriate community resources, while promoting health, wellbeing, and person-centred outcomes. You will contribute to assessments, support planning and care management activities in line with legislation, including the Mental Capacity Act, and Council policies.
