Job description

To providea high level of clinical expertise, to support and facilitate individualisedpatient centred care to patients with a life limiting illness and to providecare and support for relatives and carers in collaboration with themulti-disciplinary team.

Tofacilitate and support the implementation of evidence-based practice andcontribute to the on-going development of the Service.

Toparticipate in a seven day per week operational service.

Main duties of the job

Demonstratesleadership through clinical expertise and specialist knowledge, delivering highstandards of personalised care.

Identifyingneeds through a holistic assessment and co-creating a care and support planwith people living with cancer and other life limiting illnesses.

Toindependently manage own caseload of patients and associated responsibilities.

Toindependently manage the role of duty nurse on rotation with colleagues.

To assess,and triage new patient referrals when required, in the absence of triagespecialist nurse.

To assess,plan, implement and evaluate all aspects of the patients care.

To providesymptom control, social, emotional, and spiritual care for the patient, family,and carers.

To providesupportive care to patients, their families, and carers.

To monitorpatients on an ongoing basis, to evaluate outcomes and adjust care plans toensure optimum co-ordination and delivery of care.

To deliverhigh standards of care directly and assist in the management of junior staff.

To developand maintain effective relationships with all members of the multi-disciplinaryteam to ensure collaborative working for the benefit of patient care.

To supportthe development and implementation of care pathways and patient information.

Tomaintain accurate and up to date patient records.

To ensurethat the dignity, safety, and confidentiality of all patients is respected andthat all patients receive the highest standards of care.

We are apatient centred charity that delivers palliative and end of life care tothousands of people each year with advanced life-limiting illnesses who live inNorthwest Surrey. The Hospice alsoprovides support to their carers and families in patient homes, in care ornursing homes, or through its In-Patient Unit in the Hospice, as well as thoughthe Bradbury Wellbeing Centre. This roleis an essential component in supporting and complementing the Hospice CommunityServices Team, ensuring that the highest standards of care are implemented andthat services meet statutory regulations and national standards.

Person Specification

Experience

  • Experience of nurse management
  • Experience of working in a multidisciplinary team
  • Knowledge of palliative care and updated clinical practice within palliative care
  • Knowledge of national, regional, and local policy in relation to quality standards, targets, and improvements initiatives
  • Experience in helping to develop clinical services
  • Multi agency working
  • Experience in research and audit
  • Proven ability to manage a budget

Knowledge and Skills

  • Experience in mentoring staff and students
  • Excellent interpersonal and communication skills
  • Ability to motivate and inspire others
  • Balance of innovative / forward thinking, analytical skills with reflective practice
  • Ability to form and maintain professional working relationships
  • Calm, organised, flexible, approachable, enthusiastic, and punctual
  • Good working knowledge and application of the MS Office packages
  • Effective time management and organisation skills
  • Able to manage difficult situations effectively
  • Advanced IT skills
  • Community experience
  • Qualifications

  • Registered Nurse on the NMC Register
  • Minimum of four years post registration experience or significant palliative or oncology experience
  • Experience of ongoing professional development
  • Advanced Communication or equivalent experience
  • Management or Leadership certificate
  • Teaching qualification
  • Advanced Clinical Assessment
  • Personal Qualities

  • Excellent interpersonal and communication skills
  • Proven change management skills and the ability to support others within the change process.
  • Ability to manage workload with competing deadlines
  • Ability to work autonomously and also collaboratively as part of a team
  • Self-motivated and confident approach
  • Motivated, flexible, and adaptable individual
  • Car driver / full driving licence
  • Have access to a vehicle for community visits and ensure appropriate insurance in place
  • Ability to work flexibly over a 7 day period

  • Able to manage a varied caseload
  • Able to work autonomously and within a MDT
  • Able to plan day to day visits and weekly reviews
  • Able to monitor caseload and manage a waiting list
  • To be able to anticipate disease progression
