This team plays a vital role in ensuring swift and effective responses to the needs of vulnerable adults, providing initial assessments and triaging referrals.

Key Responsibilities :

• Provide timely, proportionate assessments for adults referred to the Front Door Team.
• Triage referrals and determine appropriate levels of intervention or support.
• Collaborate with other teams, services, and external agencies to ensure a holistic approach to service users.
• Work with individuals and families, assessing their needs and implementing care and support plans.
• Ensure safeguarding principles are applied consistently, with a focus on risk assessment and risk management.
• Maintain accurate case records and comply with statutory responsibilities.
