Job description

Job summary
The purpose of the role is to assist the clinical team in the service and delivery of the care management of the practice population. The post holder works under indirect supervision of the clinical team and undertakes task and duties delegated by the clinical team or a suitably qualified regulated professional. They will work collaboratively with the general practice team to meet the needs of patients, following policy and procedures.

Part-Time - 30 hours per week

Main duties of the job
Clinical skills health and well-being
Provide high quality patient care and treatment.

Participate in setting standards for care and auditing clinical outcomes.

Promote, participate and maintain a safe comfortable and therapeutic environment.

Participate in developing strategies, promote health programs for patients and carers and facilitate screening clinics.

Take responsibility for the safe storage and administration of drugs and equipment.

Provide nursing care in a variety of settings by holistic assessment, implementation and evaluation of programs of care as appropriate.

Participate in setting and monitoring standards to ensure effective care ensuring delivery of evidence based practice.

Support the practice in delivering or participating in primary care research and research studies, in accordance with relevant governance requirements and your training.

Recognise and escalate patient concerns.

Urinalysis and preparation of specimens for investigation by the pathology laboratory

  • Perform new patient health checks, NHS health checks and assist with the annual LTAR management
  • Assist with QOF system
  • Provide injections in line with competencies
  • Assist with the annual Flu programme
  • Provide home visit for housebound patients
  • Measure and record physiological measurements in routine presentations: Blood pressure, Pulse rate and rhythm, Temperature, Height and weight - body mass index, Visual acuity, Venepuncture, Smoking cessation, ECG


About us
We are a large practice with a patient list size of circa 15,400. We are a Partnership of 5, with 11 Salaried GPs, a nursing team of 5, an experienced managerial team of 6 (Practice Manager, Business and Finance Manager, Deputy Practice Manager, Patient Services Managers, Medicines Managers) and a friendly group of administrative and reception staff. We are a training as well as a learning practice and regularly have 6 Specialist Trainees, a Foundation Year 2 doctor and undergraduate students based here. We also offer learning opportunities to other grades and have a large apprenticeship programme operating for clinical, managerial and administrative staff. We are active in our local, supportive, Primary Care Network and have excellent relationships with external stakeholders.

Delivering a Quality Service
Firsway Health Centre aims to deliver the highest standards of care to its patients at all times. This post will be integral to the delivery of that aim and the appointee will be expected to:

Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Deliver care according to the NSF and the National Institute for Clinical Excellence (NICE) guidelines and evidence-based care.

Participate in the management and review of patient complaints, and identify learning from clinical incidents and near-miss events using a structured framework.

Understand and apply legal policy that supports the identification of vulnerable and abused children and adults, being aware of statutory child/vulnerable adult health procedure and local guidance.

Work within policies relating to domestic violence, vulnerable adults, substance abuse and addictive behaviour, and refer as appropriate.

Person Specification
Qualifications
Essential

  • NVQ Level 2 or 3 in Health and Social Care (or equivalent)/ Care Certificate
  • Experience of phlebotomy (blood-taking).
  • Previous experience working in hospitals, nursing homes or primary care.


Desirable

  • Experience of working with the general public, preferably within a service environment.
  • Good administrative and organisational skills, with the ability to prioritise work.
  • Experience of using computers, with good keyboard skills.
  • Experience of using EMIS.
