Job summary

We are looking for an Advanced Clinical Practitioner (ACP) to work as part of a new team at Ranworth PCN.Ranworth PCN is part of the national PCN pilot looking at new ways of working in primary care. We welcome applications on a full and part-time basis.

Main duties of the job

In line with the Fuller report, theproactive ambulatory care model is part of a broader shift within the NHS toward more community-based, patient-centred care that aims to keep individuals out of hospitals and support them in managing their health conditions more effectively at home.

About us

Ranworth Primary Care Network, is comprised of Caradoc Surgery, Ranworth Surgery in Clacton-on-Sea, Green Elms Health Centre in Jaywick and Kennedy Way Medical Centre in Holland on Sea. We have a diverse patient population, of around 32,000 and all practices work on SystmOne.

• Advanced clinical skills, including the ability to assess, diagnose, and treat a wide range of medical conditions, with a focus on long-term conditions and proactive care
• Post graduate diploma or degree (Advanced Practice qualification)
• Primary care experience
• Minor Illness qualification
• Experience of chronic disease management
• Experience of prescribing and undertaking medication reviews
