We areseeking a highly motivated Clinical Pharmacist to join our team in NorthLiverpool PCN. As a Network Pharmacist, you will play a crucial role inensuring the safe and effective use of medications, providing pharmaceuticalcare and expertise to patients, and collaborating with healthcare professionalsto deliver exceptional patient outcomes. Main duties of the job

Responsibilities

Conductcomprehensive medication reviews and assessments to identify and resolvemedication-related problems for patients.

Collaboratewith physicians and other healthcare professionals to develop and implementpersonalised medication therapy plans.

Providepatient education on medication use, potential side effects, and druginteractions.

Monitorand adjust medication therapy based on patient response and laboratory values.

Conductmedication reconciliation and identify discrepancies to ensure accurate andup-to-date medication lists.

Participatein multidisciplinary meetings to discuss patient care and optimise treatmentoutcomes.

Provideeducation sessions for GPs, practice staff, other health professionals andpatient groups.

Stay upto date with current literature and guidelines to provide evidence-basedpharmaceutical care.

We have a large pharmacy team consisting of 14 pharmacists and 9 technicians. Person Specification

Qualifications

Essential

  • Master of Pharmacy degree from an accredited institution.
  • Active pharmacy registration with GPhC
  • Good knowledge of pharmacology, therapeutics, and medication management.
  • Excellent communication and interpersonal skills.
  • Ability to work well in a fast-paced environment and handle multiple priorities.
  • Non-medical Prescribing qualification desirable or an interest in working towards obtaining the qualification
