Responsibilities

• Deliver daily rehabilitation interventions to facilitate patient recovery and discharge planning.
• Work within a multidisciplinary team to support patients transitioning from hospital to care homes and eventually back home.
• Utilise expertise in elderly care, orthopaedics, neurology, falls prevention, and discharge planning to improve patient outcomes.
• Assess and adapt treatment plans to meet the evolving needs of patients in a 28-day care home setting.
• Document patient progress and collaborate with healthcare professionals for seamless transitions in care.
