Job description

Overview

An exciting opportunity has arisen for a band 6 to join our new children's hospital at home community team service at Grimsby. The post holder may be required to work in Scunthorpe. The post holder must be an enthusiastic and positive individual, with a can do attitude and demonstrate experience as a paediatric community nurse.

Responsibilities

  • Lead with the hospital at home team and work in partnership with other multi disciplinary teams; identify and develop clinical changes to working practice and explore different ways to enhance service provision.
  • Provide daily leadership and clinical support for the community and service users (patients, parents / carers, primary and secondary care).
  • Work jointly with other team members to influence the overall efficiency and development of paediatric community nursing from hospital to home.
  • Work with managers to achieve service improvements and develop working practices.
  • Operate within the NMC Code of Professional Conduct and monitor professional conduct within the paediatric community team.
  • Support the 7-day service, including bank holidays, with operating hours from 8am to 6pm Monday to Sunday.
