The Role

As a Community Nurse , you'll support patients undergoing Automated and Manual Peritoneal Dialysis, ensuring safe, effective, and compassionate care at home for both adult and paediatric patients. Your responsibilities will include :

• Connecting and disconnecting PD machines during home visits
• Administering IV medications and monitoring vital signs (BP, weight, etc.)
• Performing catheter site care and dressing changes
• Managing infection control, stock, and medical waste
• Coordinating with hospital PD units for additional clinical support
• Providing education and reassurance to patients and families
