Overview

The Neighbourhood Nurse Lead is responsible for managing a community nursing team including the district nursing team in a neighbourhood.

Responsibilities

  • Provide clinical leadership, expert clinical advice, and supervision to community nurses.
  • Support the management of patients with complex health and social needs on the district nursing caseload to enhance patient experience through various pathways.
  • Work in partnership with service users, General Practitioners, Social Care, and the voluntary sector to promote a seamless service.
  • Provide proactive case management to prevent unplanned hospital admissions; assess, diagnose, and prescribe in partnership with General Practitioners.
  • Advocate for patients and act as the lead clinician for Health and Social Care integration.
  • Ensure patient safety and deliver high-quality care.
  • Engage in data analysis to identify people who would benefit from case management and monitor outcomes.
  • Assess, diagnose, and prescribe in partnership with GPs; empower carers and family members as partners in care.
  • Design and monitor care plans for patients cared for by integrated community teams and other specialists as appropriate.
  • Makes referrals to other specialists where indicated; independently prescribe in line with relevant guidelines.
  • Utilise technology such as telehealth and telecare to enhance patient outcomes.
  • Lead on implementing the Chief Nursing Officer Strategy and align practice with national commitments and the 6Cs framework.
  • Be the Patient Safety Champion, identify and manage adult safeguarding incidents with Social Care.
  • Conduct regular nursing care audits and participate in joint visits with team members or multi-professional teams.
  • Role model Trust core values and behaviours within the community nursing team.
  • Facilitate multidisciplinary supervision groups and drive continuous improvement in nursing practice.
  • Evaluate practice against evidence and benchmarks; produce data for dashboards and monthly reporting.
  • Maintain accurate and contemporaneous patient records in line with Trust and NMC standards; record communications with other professionals.
  • Develop outcome-focused care plans with clear timeframes and review dates; review and audit patient records for compliance.
  • Record meetings and communications regarding staff management and performance with HR support.

Qualifications and Requirements

  • Broad understanding of the application of research to practice; sound clinical skills; flexible approach; commitment to high-quality community care; be an effective change agent.
  • Full UK driving licence and daily access to a car is essential.
