Care Coordinator
Spencer Clarke GroupHarlesden, Greater London, EnglandA NHS Trust we are currently working with are seeking a Band 6 Care Coordinator to join their Early Intervention Service based in Harlesden. You'll be required to carry out psychiatric assessments, care planning, risk assessments, participate in MDT meetings within the Early Intervention Service.
Location : You will be expected to carry out home visits as well as see patients at the office in Harlesden.
Rate of pay : £27p / h Umbrella
Working Hours : Monday-Friday, 9am-5pm
Contract : 3 month initial contract with the view of extension
Patient age range : 14 -35years
Requirements :
- Registered Mental Health Nurse or Social Worker with a professional registration.
- Experience working within a community mental health team.
- You should have an understanding of psychosis including interventions used to treat this mental health condition.
