We are seeking a dynamic, self-motivated, enthusiastic nurses, looking for the opportunity to develop their nursing career working within one of our integrated community nursing locality teams in Walsall.
The role involves the delivery of holistic nursing care to patients within their own homes, care homes and clinic environments.
You will provide high quality clinical care, assessment and treatment for patients, actively promoting self-care and promoting independence.
You will be an excellent communicator, have the ability to work autonomously whilst also being part of a team working with an integrated approach to NOTE, WE MAY CLOSE THIS VACANCY EARLY IF WE RECEIVE HIGH NUMBERS OF APPLICATIONS.
Main duties of the job
To work within the 'Place Based Teams Care Group' within one of the integrated community nursing locality teams.
To undertake clinical assessments, implement and evaluate treatment plans, supporting patients in such areas as wound care, catheter, continence care and End of Life care. Demonstrating evidence based / best practice ensuring quality service delivery and appropriate admission avoidance. Support to junior and untrained members of staff within the team.
To adhere to Walsall Healthcare NHS Trust policies, procedures and protocols to promote the quality of patient experience in the community, ensuring the highest standard.

About us
Walsall Healthcare NHS Trust is an integrated Trust and the only provider of NHS acute care in Walsall, serving a population of 286,700, providing inpatient and outpatient services at the Manor Hospital as well as a wide range of services in the community. Walsall Manor Hospital houses the full range of district general hospital services under one roof. The £170 million development of our Pleck Road site was completed in 2010 and the continued upgrading of existing areas ensures the Trust now has a state-of-the-art Critical Care Unit, Neonatal Unit, Obstetric Theatre, and Integrated Assessment Unit facilities. Work is also well underway on its multi-million pound new Urgent and Emergency Care Centre which will house a new Emergency Department (including Children's Emergency Department), co-located Paediatric Assessment Unit, Acute Medical Unit and Urgent Care Centre. Covering Walsall and beyond, our multidisciplinary services include rapid response in the community and home-based care, so that those with long-term conditions and the frail and elderly, can remain in their own homes to be cared for wherever appropriate.

Job responsibilities
To be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care, within Scope of Professional Practice.
Demonstrate high level of judgemental / analytical skills when interpreting and performing patient assessments and care planning.
Maintain accurate and up to date nursing records, maintaining patient confidentiality, and statistical records as required by the service.
Negotiate and agree with Patient, Carers and other care professionals, individual roles and responsibilities with regard to actions to be taken and outcomes to be achieved, referring on to other services or professionals as appropriate.
Challenge professional and organisational boundaries, identifying areas for skill / knowledge development and apply these to practices to provide continuity and high quality patient centred health care.
