Overview
Are you an experienced Registered Nurse looking to join a friendly and motivated Out of Hours (OOH) Community team? We are a supportive multi-disciplinary team delivering high quality care to housebound patients, in partnership with primary and secondary care, covering moor-to-sea, coastal and Torbay districts. Joining our team as Band 5 Registered Nurse, you will provide responsive, planned and unplanned support to patients requiring urgent nursing within their own homes and residential care settings.
Responsibilities
- Work in small teams of two (you plus a Healthcare Support Worker) and work in partnership throughout your shift, including shared driving. Trust-provided Pool Cars are available for work transport to deliver planned and unplanned care.
- Support patients and their carers in partnership with HCSWs; treat post non-injury or minor injury falls, including wound dressings, catheterisation, bowel care and treatment escalation plans.
- Manage Community Nurse Equipment (e.g., scales, bladder scanner, Doppler).
- Guide and mentor junior staff and students; escalate concerns to team leaders or Community Nurse Lead.
- Generate prescriptions and order pressure-relieving equipment using available IT systems.
- Promote self-care and health education during visits to increase understanding and independence.
- Undertake Specialist Nursing and Continuing Care assessments (with appropriate training).
- Work independently and as part of a team, managing your own clinical caseload on a day-to-day basis.
- Ensure confidentiality and data protection; record, retrieve and access information in various formats.
- Communicate timely and effectively with the team manager and other team members on professional matters; liaise with patients / relatives who may be stressed or challenging.
- Provide empathetic, motivational, and negotiation skills where consent or cooperation is required; participate in best interest decisions as needed.
Qualifications and Requirements
Registered Nurse (Band 5) with current UK registration.Ability to work independently and as part of a team; manage own clinical caseload.Strong communication and interpersonal skills; ability to provide and receive complex, sensitive information with empathy.Willingness to work in a community setting and travel between patient sites as part of a small two-person team.About Us and Benefits
