Community Nurse - Mildenhall (+Brandon) INT

An exciting opportunity for a Community Nurse has arisen in Mildenhall (and Brandon) Integrated Neighbourhood Team. This INT team is one of a total of six INT teams. The other INT localities are based in Bury Town, Bury Rural, Haverhill, Sudbury and Newmarket.

The INT accommodation 'Hubs' are the base for a wide range of other health professionals and agencies including Adult Social Care, social prescribers, GPs, police and mental health team members so we are able to work together in a fully integrated way to ensure the best care is delivered to the patients within our localities.

The successful applicant / s will join a friendly team of community nurses, community matrons, therapists, social prescribers, social care staff, INT coordinators and local administrators, supported fully by a locality Team Manager, and Localities Lead.

You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi-disciplinary team and across the West Suffolk Alliance. You will be assessing the patients by carrying out home visits and planning and implementing a patient's individualised care plan.

Service integration with Virtual Ward (Hospital at Home) and the Early Intervention Team will be part of the role daily, working fluidly with these other services.

Main duties of the job

Within the INT localities you have access to a range of treatment facilities and may be involved in delivering nursing care in specialist clinics, such as the leg ulcer and doppler clinics. You will be using a wide range of community nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience of IV administration, cannulation, ECG recording, palliative care, wound management and catheter management will be an advantage.

This will be the perfect opportunity if you would like a role where you can use a wide range of nursing skills, or learn new skills specific to patient need. You are able to work independently, but also with the support of a close-knit team, who are hugely experienced and supportive. You have the privilege of working with patients in their own home environment to improve their health, alongside our service integration teams such as Virtual Ward (Hospital at Home) and our Early Intervention Team (EIT).

The role suits both newly qualified as well as experienced nurses. We can also accept applications from Return to Practices nurses (University course place dependent).

You will be able to develop your role in accordance with areas of clinical interest and to progress in your career, with expert training and education available to support this.

Key Tasks & Responsibilities :

  • To assess, plan and deliver healthcare for patients, for example with long term conditions, palliative care and rehabilitation needs, to achieve quality of life and independence where possible.
  • To work within the integrated team to facilitate early discharge from hospital and working alongside the Virtual Ward (Hospital at Home) team.
  • To work within the integrated team to prevent unnecessary admission to hospital.
  • To work with all health care professionals, and statutory / non-statutory agencies to provide a seamless, integrated service to our service users.
  • Communication :

You will be communicating with patients, their relatives, carers and colleagues and you will need to use communication techniques to motivate, negotiate or persuade internal or external stakeholders. You will use a range of verbal and non verbal communications and be able to show empathy and understanding.

  • Planning and organisational Skills :
  • The post holder will :

  • Actively contribute to multidisciplinary team meetings and other meetings.
  • Support all team members within the integrated team, respecting them, their roles and contributions.
  • Be responsible for safe use and maintenance of equipment and supplies and be aware of budgetary constraints and work within these.
  • Be accountable for own professional actions as determined by the Nursing and Midwifery Council code.
  • Be accountable for work you have delegated to others within the team, ensuring staff are competent to undertake delegated work.
  • Ensure adherence to Trust policies, and procedures.
  • Be aware of Trusts behaviour framework and ensure behaviours are embedded in role.
  • Ensure Trusts behaviour framework is utilised when managing and supervising staff, and that behaviours are embraced and embedded within the team.
  • Take part in clinical supervision as per Trust Policy.
  • Demonstrate own duties to students, new starters and / or less experienced staff.
  • Provide mentorship for students undertaking pre / post registration course.
  • Provide Preceptorship to new staff as appropriate.
  • Contribute to clinical audit as required.
  • Complete risk assessments and incident forms as required.
  • Person Specification

    Education and qualifications

  • RN Qualification (Part 1 of register) or equivalent.
  • oNMC registration required or NQ Nurse awaiting registration
  • Experience and knowledge

  • Good communication skills which demonstrate tact, diplomacy, empathy and sensitivity
  • Able to communicate effectively using the English language (written and verbal)
  • Highly motivated
  • Able to provide high standards of care
  • Ability to use own initiative
  • Ability to manage stressful situations
  • Ability to be focussed undertake patient care and input accurate data / patient records
  • A team player
  • Able to communicate effectively where there are barriers to understanding
  • IV Administration / cannulation
  • ENB 998 / mentorship preparation or equivalent
  • Mentorship qualification is essential for the post, but not essential at interview.
