Qualified Social Worker - Complex Care Team

We are looking to recruit a Qualified Social Worker in the Complex Care Team within Islington. The successful candidate will be working in a well-structured, organised team who are growing their social worker staffing levels.

The responsibilities of the Social Worker includes :

Carry out complex needs assessments for adults and their carers.

Create, implement, and review person-centered care and support plans.

Manage a complex caseload with a high degree of autonomy.

Lead complex safeguarding inquiries and multi-agency risk assessments.

Ensure full compliance with all relevant legislation, including the Care Act 2014 and Mental Capacity Act.

Work collaboratively with a wide range of health and social care professionals.

Advocate for service users and their families.

Provide advice and support to clients and their network.

Maintain accurate and timely case records and reports

The ideal candidate will have :

Social Work England registration

Eligibility to work in the UK

Social Work qualified (MA / BSc / DipSW / CQSW / MSc / BA)
