Job summary

Following a retendering process, the children'sservice is due to be transferred to the new provider on the 1st of April 2025.Given this post sits within the transferring service, in line with thelegislative requirements, the successful candidate will be part of the TUPEtransfer along with other colleagues.

An excellent opportunity has arisen for enthusiastic, motivated,inspirational and innovative School Nurses to join our experienced SchoolHealth team within Children and Family Health Surrey. The post will be at Mole Valley borough of Surrey, based at Leatherhead Clinic.

You will be a highly skilled clinical practitioner, withleadership and excellent interpersonal and communication skills. You will alsohave a strong approach to teamwork whilst also demonstrating the ability towork on your own initiative. You will have a good understanding of children,young people and their families and the ability to deliver the Healthy ChildProgramme to a diverse population across specific geographical areas of Surrey.

Main duties of the job

Works collaboratively with the skill mixed school nurse team toensure equity of service provision, promoting good practice and the sharing ofresources.

Prioritises own work in line with areas of highest risk or clientgroups with greatest health need, delegating other work to the wider skillmixed team whilst providing the team with supervision and support

Build and develop good working relationships with named schoolsand the wider multi-disciplinary team.

Work collaboratively with all relevant agencies toinfluence / develop policies which will enhance health

Assist in the planning and may participate in the delivery ofenuresis clinics and continence assessments

Design deliver and evaluate planned programmes of health promotionto CYP either through one to one contact, group work or through delivery ofschool based PSHE lessons

Assist in the planning and may participate in the delivery ofenuresis clinics and continence assessments

Identify and support CYP who have unmet health needs, providesupport and targeted interventions and where necessary refer to specialistservices

Undertake health needs assessments for children who are lookedafter or from hard to reach group

Attend child protection conferences / core groups as required andparticipate in safeguarding supervision

Purpose

The post holder will be a member of the 0-19 public health teamdelivering a service for children, young people and their families. Supportedby the clinical team lead, the post holder will carry continuing responsibilityfor the assessment of health needs, including the development, implementationand evaluation / audit of evidence based interventions. She / he will takeresponsibility for ensuring the quality and standard of care and delivery of aneffective school nursing service to improve outcomes for the local school agedpopulation. The post holder will participate in delivering a universal coreservice, and provide targeted interventions, to address health inequalities,where necessary referring to specialist services. This role will also includesafeguarding responsibilities, supporting the delivery of the schoolimmunisations programme, mentorship of students, line management of a skillmixed 0-19 team and staff appraisals.

Principal duties to include :

Responsible for organisation, prioritisation and planning ofhealth promotion activities, by assessing local health needs and schoolprofiling promote key health topics to meet All our health 2016 DH targets

Make contact with children, young people (CYP) and their familiesin a variety of community settings schools, home, children centres, youthclubs

Assist in the preparation, planning and delivery of the schoolbased childhood immunisation programme in accordance with local policies andprocedures following appropriate immunisation training

Following specific training be part of the CHAT HEALTH schoolnurse text advice service

Following specific training provide Clinic in a box sexual healthcontact with young people

Take appropriate action in cases of suspected or actual childabuse, working within Surrey safeguarding childrens code of practice andprocedures.

Attend child protection conferences / core groups as required andparticipate in safeguarding supervision

Facilitate group clinical supervision as per organisation policy

Ensure that clear concise reports are written and that recordkeeping guidelines are adhered to. Participates in the monitoring of records ofothers within the 0-19 team.

Assist in research and design / completion of service audits

Actively participate in steering groups to develop SN practiceguidelines

Supervise junior member of the skill mixed 0-19 team as directedby the clinical team lead and undertake junior staff PDR appraisals and directline management.

Assess practice competencies of junior staff members, contributeto their training and development needs

Participate in meetings as appropriate

Be prepared to be flexible within contracted hours

In accordance with the needs of the service, undertake otherduties as required.

Communication skills (factor 1)

Be skilled at adapting language and professional approach to avaried client group

Be proficient in designing, producing and presenting healthpromotion sessions to differing sizes of groups, eg PSHE lessons, schoolassemblies, parent evening, school teacher training sessions

Be competent in writing concise accurate contemporaneous clientrecords using approved CFHS templates

Be competent at making referrals to the wider multi-disciplinaryteam

Demonstrate ability to produce child centred reports for CYP whoare subject to safeguarding plans or CIN / LAC reviews

Contribute ideas to discussions at school nurse forums and teammeetings

Lead at or chair team meetings when necessary in the absence ofthe clinical team leader or practice teacher.
