Responsibilities

• Facilitate patients' safe and efficient discharge from the hospital by collaborating with a multidisciplinary team.
• Assess patients' needs and develop personalised discharge plans.
• Communicate effectively with patients, families, and carers to ensure a clear understanding of discharge processes.
• Coordinate with healthcare professionals, social workers, and external agencies to arrange necessary post-discharge support.
• Monitor and evaluate patient outcomes post-discharge, ensuring continuity of care.
• Maintain accurate and up-to-date patient records to support seamless service delivery.
