Role

Support required with accessing the outside.

Preferred Days / Times

Support required in the morning, time and day to be arranged.

Tasks / Duties

• Support needed for the client to access the community e.g. go to parks / green spaces, go for a coffee and / or go shopping.
• PA will need to have access to a vehicle and a clean, full, UK driving licence.
• Some toilet assistance is required when out in the community.
• Reassurance may be required to be given at times when out and about.

Client Main Diagnosis

Manual Handling

May need support to transfer on and off the toilet from mobility scooter or wheelchair when in the community depending on equipment in place in public toilets. In terms of using a car, the client can lower themselves into smaller cars and uses the grab handle above the window for support.

Communication

The client can be difficult to understand however as a person gets to know them it gets easier. They become frustrated when not understood.

Client Enjoys

The client enjoys going shopping, going to green spaces e.g. parks. They find being in crowded areas stressful.
