Qualified Social Worker, Mental Health Team

Sanctuary Personnel, an innovative and committed recruitment agency has a new permanent position available for a Social Worker within the Adult Mental Health Team to work full time based in Harrow.

The salary for this permanent Social Worker job is up to £42,459 per annum.

• To work in a strengths-based way with adults and their families and carers, including those who are terminally ill. This includes working with multi-disciplinary colleagues, providing advice and information, working with community supports and liaison with other agencies, seeking the help of other professionals as necessary.
• To have person centred conversations with people and carers in a timely way evaluating the level of risk and an evidence-based analysis of need whilst promoting independence in accordance with agreed criteria and divisional policies.
• To assess for and provide equipment and minor adaptations to promote independence, and to monitor these as necessary, seeking advice where appropriate.
• To maintain and take responsibility for effectively managing a personal caseload including complex cases in consultation with the Team Manager.
• To work as a member of a multi-disciplinary team liaising with other professionals including health and the voluntary sector ensuring that there is the fullest co-operation with other team members, to provide an easily accessible service geared to meeting people’s needs.
• To ensure, along with other colleagues in other parts of the department, that people and their carers are involved in all decisions and advocates are recommended where appropriate.
• To assist in the planning of services in consultation with other professionals, to enable people to remain as independent as possible.
• To prepare and present reports in clear and concise and logical way and to a high standard, which contribute to multi-agency and multiple disciplinary conferences and meetings and court hearings and attend these forums.
• To seek advice and guidance on matters that need resolution and report shortfalls in services to line managers.
• To offer advice to other works in the multi-disciplinary team where a social worker has a particular level of skill or knowledge.
• To undertake any other duties that may be required commensurate with the general level of responsibility of the post.
