Overview
The role involves attending weekly Multidisciplinary Team (MDT) meetings and providing comprehensive administrative support. Responsibilities include preparing patient proformas, organising meeting materials, logging action plans, preparing and circulating patient lists, maintaining accurate records of meeting outcomes, data validation, facilitating MDT service improvements, and liaising with other Trusts for necessary patient information. The Senior MDT Coordinator role coordinates MDT meetings and manages patient pathways to ensure efficient delivery of care, with leadership responsibilities, service development, and involvement in projects and service improvement initiatives.
Main Responsibilities
- Coordinate and manage multidisciplinary team (MDT) meetings and patient pathways to ensure timely and efficient care
- Prepare information for MDT meetings and ensure clinics and admissions are well-prepared by collating investigation reports and providing clinicians with necessary information before patient appointments
- Maintain accurate pathway tracking and communication with clinical teams and stakeholders
- Log action plans, monitor waiting lists and service capacity, and escalate issues as needed to ensure access and quality targets are met
- Communicate complex or sensitive information clearly with the multidisciplinary team, patients, families, visitors, carers, and other clinicians (e.g., GPs). Provide patients with appointment details and non-clinical care-related advice
- Resolve complex queries using analysis and judgment; escalate to clinical staff when necessary and support administrative staff with escalated queries
- Utilise hospital information systems (including Epic) to produce accurate correspondence for patients, GPs, and other care providers; receive and register referrals in line with the Trust’s Elective Access Policy
- Manage appointment and diagnostic bookings or rescheduling according to Trust policy and resolve complex scheduling issues escalated by the admin team
- Identify potential pathway delays or breaches and proactively take steps to prevent them (e.g., chasing diagnostic tests and results)
- Support the MDT process by preparing information and ensuring data quality

The Senior MDT Coordinator plays a pivotal role in coordinating MDT meetings and managing patient pathways, with leadership, service development, and active contribution to projects and service improvement initiatives. This role supports the Service Manager in operational management and strategic planning to enhance patient outcomes and pathway efficiency.
