You’ll play a key role in shaping high‑quality medicines optimisation, providing quality medication reviews and medication reconciliations across our three well‑established practices, supporting around 32,000 patients across Tadcaster Medical Centre, Sherburn Group Practice and South Milford Surgery.

You will work closely alongside Practices forming an integral part of the clinical teams whilst also working alongside the PCN Team as part of a PCN Hub. The role will sit alongside other Pharmacists and Pharmacy Technicians.

The right candidate should have a passion and drive for providing high quality patient‑centred care both in person and via telephone. A prescribing qualification is essential for the role. The post‑holder will be required to enrol on the General Practice Pharmacist Training Pathway (CPPE) if not already completed.

Note: The role is eligible for a NEST pension only.

  • Supporting medicines management in care homes.
  • Reviewing and actioning prescribing safety alerts.
  • Contributing to QoF, DES, and IIF outcomes.
  • Leading clinical audits, sharing insights and driving improvements.
  • Supporting the day‑to‑day management and development of the pharmacy team, including appraisals and clinical supervision.
