Overview

An exciting opportunity has arisen for skilled and enthusiastic 16 to 25 Community Mental Health Link Worker within the Hammersmith and Fulham Mental Health Integrated Network Teams (MINT).

Responsibilities

  • Provide recovery focused interventions that utilise community assets and the multi-disciplinary team to support young people experiencing mental health challenges.
  • Specialise as a Link Worker in signposting to and supporting referrals to identified appropriate community resources, fostering autonomy and promoting independence.
  • Assist individuals in building on their strengths, gaining skills and confidence in collaboratively identified areas of support, and escalate concerns when they arise.
  • Work effectively both independently and with colleagues from a range of backgrounds, demonstrating resilience, motivation, and adaptability in an ever-changing environment.
  • Spend part of the role working closely with CAMHS to support young people in accessing vital community services and ensure a smooth transition into adult services.

Context and Service Model

The community mental health framework for adults and older adults (2019) set out a vision for local services that reflect the needs of the local population. West London NHS Trust is piloting Mental Health Integrated Network Teams (MINT), offering needs-based, episodic interventions integrated with primary care.
