Job summary

St. Luke's Hospice is a charity rooted in the local community and dedicated to providing compassionate care to individuals facing incurable diseases. Our goal is to preserve dignity and provide patient choice during the final stages of life.

We are now exploring an opportunity for an experienced and devoted Rapid Access to Discharge Service Clinician to join our team. This provides a rewarding opportunity for someone who is passionate about delivering high-quality palliative care services.

Main duties of the job

As our Rapid Access to Discharge Service Clinician, you will be instrumental in coordinating the safe and timely discharge of patients from hospital to their preferred place of care. Working in conjunction with a multi-disciplinary team, you will offer support to patients, relatives and healthcare professionals throughout the discharge process.

You will need to have relevant palliative care experience to be successful in this role and because you will be working out in the community, you need a drivers licence and use of a car / vehicle.

Job responsibilities

Key Responsibilities include :

  • Developing an effective discharge plan in collaboration with the hospital, GP and community services. Ensuring Hospice rapid access funds are applied appropriately
  • Providing necessary care, support and advice to patients, family members and other healthcare professionals & that patients are reviewed in a timely manner.
  • Contributing towards on-going quality improvement of palliative care services by participating in audit and research activities
  • Working cohesively within a multidisciplinary team to ensure effective communication and optimal patient care

Person Specification

About you

Essential

  • Registered Nurse Qualification and NMC pin number
  • Confident IT skills
  • Excellent written and verbal communication skills
  • Flexibility and willingness to work on rotating shift patterns
  • Demonstrated experience as a Clinician within a palliative care setting
  • Strong interpersonal and communication skills
  • Excellent organisational skills with the ability to manage a high caseload
  • A commitment to the values and mission of St. Luke's Hospice.
