Job description

Full-time, Part-time and Bank Advanced Clinical Practitioner £55,157 to £68,544, yearly salary increments, NHS Pension provider, rate enhancements up to 70% for evening, weekend and overnight working.

gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.

We have fantastic opportunities for Advanced Clinical Practitioners (ACP) to work in Out of Hours and Urgent Care across Greater Manchester.

Based at an individual service location or multisite portfolio role across a number of services, if desired. Flexible full time, part time and bank positions are available for Advanced Clinical Practitioners predominantly evenings and weekends.

Whilst working as an ACP, you will work alongside a supportive multidisciplinary team that includes Nursing, Allied Health Professionals and GPs, who have a real commitment to continuous improvement, shared learning and best practice, while being passionate about providing patients with the highest possible quality care. The ideal candidate must be able to see 3 months + children.

The successful candidate will have experience working in primary and urgent care as an ACP, you will be expected to provide care via face to face, telephone triage and home visits.

The post holder will hold an MSc in Advanced Clinical Practice or Credentialing qualification (or equivalent) and a Non-Medical Prescribing qualification. You will be able to work within the four pillars of practice.

Main duties of the job

Out of Hours Clinical Hub

Our out of hours clinical hub is open 24 hours a day and 7 days a week. The ACP role involves providing face to face care, home visits and telephone triage, assessment and management for patients calling 111 with primary care conditions. You will support junior clinicians on site, prioritise incoming calls and support direct face to face booking across urgent care.

City Health Centre Walk-in Service

City Health Centre Walk-in Service is situated above Boots on Market Street, which offers a great selection of cafes, restaurants and shops with excellent transport links. We operate 08:00 to 20:00 and 7 days per week. The ACP role involves face to face assessment, treatment and management of primary care presentations. Patients self-present to the walk-in centre for treatment. Referrals are also made by NHS 111, which are booked directly into appointment slots.

Oldham Urgent Care and Primary Care Streaming Service

Our services operate between 08:00 to 00:00 and 7 days per week. The ACP role is face to face assessment and management of primary care patient presentations. Patients are either streamed to the Primary Care Streaming Service or through the NHS 111 direct booking pathway to the Oldham Urgent Care Centre. The role will be hybrid working across both sites, it also encompasses NHS 111 telephone assessment and triage which can be undertaken within the clinical hub.

Job responsibilities

Clinical Practice

  • Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
  • Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
  • Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
  • Refer and discharge safely ensuring that safety-netting advice is offered.
  • Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.

Communication

  • Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
  • Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.
  • Anticipate barriers to communication and take action to improve communication.
  • Maintain effective communication within the organisation and with external stakeholders.
  • Ensure awareness of sources of support and guidance and provide information in an acceptable format to all patients, recognising any difficulties and referring where appropriate.

Non-Medical Prescribing

  • If you are employed in a prescribing capacity, you must only prescribe within your scope of practice and comply with the gtd healthcare NMP governance registration processes.
  • You must maintain your knowledge and skills and prescribe within your scope of clinical competencies.
  • The non-medical prescriber is required to comply with organisational NMP governance processes and adhere to local and national prescribing guidance.

Delivering a Quality Service

  • Recognise and work within own competence and professional code of conduct as regulated by the NMC/HPC etc. Understand own role and scope and identify how this may develop over time.
  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.
  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.
  • Ensure professional revalidation requirements are met.
  • Deliver care according to NSF, NICE guidelines and evidence-based care.
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation.
  • Initiate and participate in the maintenance of quality governance systems and processes across the organisation and its activities.
  • In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate.
  • Support and participate in shared learning across the organisation and wider organisation.
  • Understand and apply guidance that supports the identification of vulnerable and abused children and adults, and be aware of statutory child/vulnerable patient health procedures and local guidance.

Qualifications

  • Registered health care professional, including first level nurse, paramedic, pharmacist or other AHP with MSc Advanced Clinical Practice or credentialing qualification or equivalent. V300 Non-Medical Prescriber (gateway dependent) dependent on professional eligibility.
  • Mentor/Teaching qualification. Clinical supervision training and experience.

Experience
