About the role
As an Intensive Case Management Worker, you will work in a psychologically informed way to establish open, trusting and high-quality professional relationships with members. You will work with members who are experiencing homelessness and facing multiple and complex needs, including but not exclusively, mental and physical health needs, substance use difficulties, multiple exclusion and social isolation. You will ensure that each member you work with has a thoughtful and detailed assessment of their needs and strengths and that they are supported with a clear plan to end their homelessness. You will hold a holistic view of a member’s journey with us as part of our ‘one worker, one member’ lead working approach.
About you
We are looking for an experienced, skilled and driven person to join the team as a Case Worker in the Intensive Case Management team. You will have experience of providing flexible, person-centred support to people. You will be joining a highly experienced, motivated and supportive team seeking to improve the quality of life of people experiencing homelessness in London.
