The Role

We are seeking a Mental Health Social Worker to join our Central Locality Mental Health Team. You will :

Undertake Care Act and carers’ assessments, safeguarding enquiries, and reviews.

Develop care and support plans that promote independence and wellbeing.

Support people with functional and / or organic mental health needs, making effective use of community resources and social care budgets.

Deliver evidence-based practice including motivational interviewing, trauma-informed care and psychoeducation.

Provide guidance on the Care Act, Mental Health Act and safeguarding procedures.

Work closely with multi-disciplinary colleagues in health, housing and voluntary services.
