Job description

Job summary

The London Home Oxygen Service (HOS) contract is responsible for circa £10million expenditure annually on oxygen service provision to people in their homes. This role will support the delivery of the London HOS agenda, working with regional and national colleagues. The post holder will deputise for the London HOS regional lead as required. The post holder will support the wider London HOS clinical teams within primary care and the acute sector, providing clinical and contract management support.'The SEL ICB Medicines Optimisation programme is responsible for £200million in medicines cost in primary care and approximately £30m expenditure on medicines in the acute sector (ICB commissioned high cost medicines). The programme includes a wide range of quality, safety and clinical effectiveness workstreams ensuring that we have a high value, innovative medicines optimisation programme in SEL which supports proactive chronic disease management, promotes self-care and empowers patients. The post holder, through the staff and structures provided, is expected to lead legal, safe and effective delivery of key integrated medicines optimisation plans which are high priority across boroughs in SEL.

Main duties of the job

As a senior pharmacist, you will work with our pharmacy leadership team, to support the delivery of the SEL ICB medicines agenda and the London Home Oxygen Service agenda. You will support the delivery of the Long Term Conditions improvement programmes, providing advice and guidance to GPs and other professional across the system. You will support and contribute to SEL clinical networks relating to diabetes, CVD and respiratory medicines pathways (including oxygen). You will provide guidance to the HOS delivery team and deputise for the HOS regional lead in regional and national meetings.

The role will lead on SEL-wide work streams, including but not limited to work streams under the SEL Integrated Medicines Optimisation Committee, the SEL Integrated Pharmacy Stakeholder Group (IPSG) and their sub-groups. The role will work in a collaborative and matrix style with the Lead Pharmacists in the Integrated Medicines Optimisation Team.

Education / Qualifications

Essential

  • Educated to masters level with additional in depth professional knowledge in own discipline
  • Current registration with General Pharmaceutical Council
  • Postgraduate qualification in clinical / community pharmacy or equivalent additional experience
  • Mandatory Continuing Professional Development to maintain fitness to practice and revalidation

Desirable

  • Member of the Royal Pharmaceutical Society

  • Experience of developing medicines optimisation services in challenging environment
  • Knowledge of developing long term conditions prevention or management services and an understanding of personalised care
  • Extensive experience of negotiating and implementing change in practice

  • Experience of leading a specialist team, service changes and developments
