Main duties of the job
You will be a vital part of a supportive, friendly and specialist team who have a great reputation for ensuring that patients, families / carers receive seamless support in the community. In this role, you will work flexibly to support and deliver care. You will undertake triage and rapid response visits, manage palliative discharges, assess and review care support, conduct full patient assessments and plan will need to have relevant palliative care experience to be successful in this role and because you will be working out in the community, you needa drivers licence and use of a car / vehicle.
