Main duties of the job
The role of the Highly Specialist Heart Failure Nurse is to provide specialist advice to support the assessment, planning and implementation of programmes of care, within the inpatient and outpatient / community setting, autonomously and within the specialist team. To carry out specialist nursing procedures, accountable for own actions, not directly supervised. Lead specialist in a defined area of nursing care. Provide specialist education to other staff and learners. Undertake research and lead clinical audits in own specialist area.
To provide comprehensive inpatient assessment and planning of care to inpatients via the referral system to facilitate appropriate treatment and contribute to reduced length of stay, displaying compassion and empathy to the patients during their hospital stay. Work within the Heart Failure team and contribute to the multidisciplinary team meeting to optimise patient care. Work within the outpatient / community setting in nurse‑led clinics to provide comprehensive assessment and treatment of patients with heart failure, actively monitor and participate within our caseload of patients.
The specialist nurse will actively participate in Virtual Ward, whereby patients’ blood pressure and weight are monitored post‑discharge to prevent decompensation.
