Job description

Healthier South Wirral is currently recruiting for a fulltime Learning Disability Nurse Practitioner.This will includeworking as a member of the multi professional PCN team, providing directsupport to people with learning disabilities.

Main duties of the job

To work as a member of the multi professional PCN team,providing direct support to people with learning disabilities living in theirhome which will include :

Workingwith their families and Care & support providers.

Providingexpertise and skills to identify the health needs and support required in theprovision of care.

Providinghealth promotion role to support and empower people with a learning disabilityto access mainstream services to ensure their needs are met in the community.

To support people with a learning disability to live asfully and independently as possible whilst respecting their rights and dignity.

About us

Healthier South Wirral Primary Care Network is made up of 5Practices. Eastham Group Practice, Civic Medical Centre, Orchard Surgery,Spital Surgery and Sunlight Group Practice. We currently have a list size ofover 50,000 patients. The PCN also have a team of ARRS staff which includesClinical Pharmacists, Mental Health Practitioners, Social Prescribers, AdvancedNurse Practitioners, Nurse Associates, Paramedics, a Trainee PA and CareCoordinators.

Job responsibilities

To provide support and work directlywith people with learning disabilities in their home, including working withtheir families and Care & Support providers

To provide day to day clinical supportto any current or future LD team members

To ensure all practice is based upon theprinciples of Valuing People, rights, independence, choice and inclusion

To work with the individual, theircarers and members of the multi professional team in the formulation,implementation and review of a person-centered care and support plan. This willinclude risk assessments and management plans that will outline the support theindividual requires to continue to live successfully within the community

To undertake learning disability healthchecks on all patients on the learning disability register aged 14 and above onan annual basis

To identify any training needs requiredfor carers -family and providers- and when appropriate contribute to thedelivery of this training to enable them to be successful in supporting theservice user and to meet their individual needs

To promote the health needs of theindividual and provide health facilitation support to them, this will alsoinclude completion of annual learning disability health checks and supportingthem in the development of quality health action plans

To support the mainstream services toensure the delivery of good quality general health care to people with learningdisabilities

To liaise with, train and support thePCN team to better meet the health care needs of people with learningdisabilities

To work closely PCN GP practices, bothGP and practice staff, in raising awareness of the health needs of people withlearning disabilities

To support the individual in attendingtheir GP to ensuring that they receive an annual comprehensive health check

To support the individual to attendhealth appointments as required and to provide information in an accessibleformat regarding and prescribed treatment

To support the individual with theirprescribed medication, to assist them with any changes to their medication andprovide a monitoring role; to report any concerns on behalf of the individualsback to the GP / -Consultant

To providesupport, monitoring and advice to individuals who have epilepsy

To oversee andcoordinate care for individuals under the Care Programme Approach, ensuringadherence to relevant policies, and to take responsibility for managing thecare of those assessed as eligible for Continuing Healthcare

To undertake acare management role when it has been identified most care required is healthcare and it is felt that there is an added benefit to the individual for anurse to undertake this role. This will include co-ordinating meetingsto review the quality and delivery of care

To undertake carers assessments toensure all needs are met to enable them to support their relative and ensureany unmet needs are highlighted

To ensure compliance with the Policy inthe Protection of Vulnerable Adults and ensure all practice actively helpssafeguard vulnerable people. This will include reporting any issues of concernas laid down in the policy and contribute to the development of adultprotection plans

To prepare and review the effectivenessand progress of the care plan with the individual and others; to initiate anyaction that may be required to assist them in achieving their choice

To provide advice, which includes bothclinical and management strategies, regarding the management of individuals whoare exhibiting challenging behavior

To organize and lead group work withpeople with learning disabilities, this may include issues such as anxietymanagement and healthy living

To participate in reviews, preparingreports for the multi-agency meetings.

To maintain a high standard of recordkeeping and case notes in line with NMC guidelines

To be aware of the implications andresponsibilities relating to the Mental Health Act -1983- 1.32 To ensurecompliance with the Mental Capacity Act

To ensure that all care plans have beendeveloped within the guidance of the Mental Capacity Act and where appropriateto involve advocacy

To undertake comprehensive nursingassessments, behavioural assessments, and any other assessments appropriate tothe individual

To work closely with staff within thecommunity teams and care and support providers to ensure a person-centeredapproach is always maintained

To liaise with members of themulti-agency teams, attending and contributing at reviews or meetings andcommunicating appropriate information as required

To ensure that any information providedregarding any aspect of their care is available in an accessible format whenrequired

To undertake comprehensive nursingassessments, behavioural assessments, and any other assessments appropriate tothe individual

To interpret and understand behaviour todevelop individual care packages
