Responsibilities

• Conduct comprehensive assessments and interventions for patients with complex needs, incorporating physical, personal, and psychological dimensions to facilitate discharge.
• Use strong clinical reasoning and positive risk-taking to support home-first discharges in line with Welsh Government policy.
• Operate efficiently within a fast-paced, unpredictable environment.
• Communicate sensitively and effectively with patients, carers, agencies, and multidisciplinary teams.
• Utilise excellent organisational and communication skills in daily duties.
• Update clinical information via computer systems for data collection and audits with precision.
• Maintain and enhance professional knowledge in accordance with HCPC requirements, with active support for seeking internal and external training opportunities.
• A valid UK driving licence is essential for reaching patient locations and ensuring timely interventions.
