Job description

Job summary

Barnet Unplanned Integrated service works primarily as an admission avoidance service reducing the number of patients sent to hospital Emergency departments. We receive referrals from GP, LAS, 111, Residential and Nursing Homes to prevent admission of patients and support immediate discharge home from the emergency departments.

Main duties of the job

To provide skilled, effective, and evidence-based nursing care to patients with complex care needs, in a variety of community settings.

To undertake health promotion and disease prevention and to provide information and support to promote and optimize positive health and to constantly champion the principles of self- care and patient empowerment, referring on as appropriate.

Plan and facilitate the safe and seamless transfer of care from the Rapid Response and Enhanced Virtual Service to appropriate other services.

Evaluate outcomes for patients in collaboration with other health and social care colleagues.

Be aware of, and act in accordance with, the NMC Code of Conduct, Standards and Guidelines.

When working in clinical areas monitor and maintain required stock levels (medicines / dressings / clinical equipment). Ensure hygiene and cleanliness standards are adhered to as per the Trust's Infection Prevention Policy.

Ensure at end of clinical shift that all clinical equipment, medicines, keys, and prescription pads are stored securely.

Ensure that all workspaces are left tidy and secured when not in use.

To have an innovative approach to practice in response to changing service needs and priorities. Acting as a change agent and facilitating the change process.

Education / Qualification

Essential

  • Registered Nurse with current NMC registration
  • BSc / PgDip Nurse Practitioner Primary Health Care / Minor injuries / Minor Illnesses
  • Qualification in Community and / or Accident and Emergency Nursing.
  • Evidence of continuing personal and professional development with up-to-date CPD portfolio
  • Community or Independent Nurse Prescribing qualification or prepared to work towards
  • Desirable

  • Evidence of continuing professional education and training at master's Level.
  • Further desirable qualifications in Primary Care / Accident & Emergency
  • Qualification in teaching or practice-based learning.
  • Independent Nurse Prescriber.
  • Management or leadership qualification
  • Experience

    Essential

  • Extensive senior experience within adult nursing services especially Primary Care and / or Accident & Emergency.
  • Proven experience of utilising expert diagnostic and clinical skills to provide autonomous, unscheduled nursing care and management of patients presenting with undifferentiated and undiagnosed conditions
  • Experience of undertaking comprehensive health needs assessment and developing complex care plans
  • Experience of assessing and providing quality care to people with complex needs and long-term conditions.
  • Track record of practice excellence
  • Experience of effectively working in collaboration, negotiating, and liaising within a multi-disciplinary team and across organisational boundaries.
  • Experience in monitoring clinical standards

  • Competent in physical examination of : ENT, eye, gastrointestinal, respiratory, cardiovascular, neurological, skin, musculoskeletal systems, and wound management.
