Job title :  Practice Nurse   Location : Balderton Primary Care Medical PracticeSalary :  Up to £48,000 Depending on ExperienceHours : Part-time (27.5 hours)Key Responsibilities : As a valued member of the team, your key duties will include : Assisting the registered manager with duties related to CQC outcomes, ensuring that compliance is maintained.Working collaboratively with the general practice team to meet the needs of the patients, ensuring practice policies and procedures are followed.Providing care for the presenting patient, treatment, chronic disease management and evaluation of care.Providing a wide range of nursing services to patients, including chronic disease management, health screenings, and immunizations.Conducting health assessments and screenings, including blood pressure checks, vaccinations, and general health checks.Supporting patients with long-term conditions (e.g., diabetes, asthma, COPD) and assist in the creation of care plans.Administering vaccinations, travel health advice, and general health education.Performing routine treatments, such as dressings, injections, and minor wound care.Supporting with the management of health promotion and disease prevention within the practice.Taking patient histories, carry out clinical assessments, and offer advice on lifestyle changes and disease prevention.To find out more about this Practice Nurse position, please call Alisha ASAP on (phone number removed) or apply for the position above to receive the full job description
