Dudley Integrated Health and Care NHS Trust (DIHC) is the first Integrated Care Provider in the country. It was launched in April 2020 following a period of development and integrates primary care with community services within the Dudley healthcare system. Our core aim is Dudley First, community where possible and hospital where necessary, and our purpose is to improve the health and wellbeing of our local population in Dudley.
The primary focus of the post holder will be to support high quality service delivery of the First Contact Mental Health Service across a range of practices within their designated PCN to the population of Dudley. The post holder will be required to engage a range of key partners and stakeholders and lead on joint working across health and social care. In particular, they will need to work closely with colleagues within DIHC, other provider organisations, local authority / Office of Public Health, and the voluntary and community sector.
