Job description

Job summary

This is an onsite part time role for 16 hours.

An exciting opportunity has arisen for clinically focused, highly motivated clinical pharmacist who wish to expand their experience by working onsite in a General Practices within the South Manchester area.

You will be enrolled on an 18-month national training program, including attainment of an independent prescribing qualification (if applicable). You will also be supported by a national network of pharmacists who are undertaking the same journey.

Main duties of the job

Using GP systems (EMIS and System 1) you will support practice sites with medicines related tasks, this may include but is not limited to :

  • Discharge support (meds rec)
  • General med reviews
  • Supporting patient requests for acute and repeat meds
  • Structured Medication reviews
  • Safety audits

    Job responsibilities

    You will work as part of a multi-disciplinary team in a patient-facing role, working with colleagues to help resolve medication-related issues, helping to utilise skill mix, improve patient outcomes and ensure better access to healthcare.

    You will support practice staff with prescription and medication queries. Help support the repeat prescription system, deal with acute prescription requests, and medicines reconciliation, providing expertise in clinical medicines advice while addressing both public and social care needs of patients.

    You will support with prescribing practices across the PCN, providing clinical leadership on medicines optimization and quality improvement.
