Care Brokerage and Placement Coordinator

This is a full-time, permanent role with hybrid working, and a salary of £27,372 - £31,169 per annum.

The Role

We are looking for a self-motivated person to join our Adult Social Care Brokerage Team. You will be working as part of a Care Brokerage Team but also can work autonomously when required.

Your Role Will Involve

  • Commissioning and procuring services for various client groups.
  • Experience of negotiation, particularly in relation to health and social care services.
  • Working jointly with Care Managers, Social Workers and Service Providers.
  • Setting up domiciliary Care Packages / Re-starts / Suspensions and ending care packages in addition to sourcing Nursing / Residential care providers.
  • Obtaining services (residential and non-residential) as indicated in a client’s support plan to meet their needs for social care services following an assessment.

  • Good communication and IT skills.
  • Experience of accurate data entry, analysis and reporting from spreadsheets and databases.
  • Ability to work well within a team.
