Job overview

A rare opportunity has arisen to join our Heart Failure Nurse Specialist Team.

We are looking for an experienced, motivated, and committed Senior Heart Failure Nurse with a passion for improving the experience and clinical outcomes of our patients, and who inspires others to provide excellent patient care.

You will be an experienced HF specialist nurse who is a NMP with a post graduate qualification in history-taking, physical assessment, diagnosis and decision making and have completed the BSH competencies or equivalent. Ideally you will have undertaken advanced communication skills training and will have experience of supporting learners in practice. We are looking for someone who is adaptable and able to work independently across the inpatient and outpatient settings.

Main duties of the job

To provide specialist assessment, investigations, treatment and advice for patients admitted to hospital with a possible diagnosis of heart failure; to improve the speed and accuracy of diagnosis, treatment and discharge.

To comprehensively manage the heart failure patient within the outpatient setting with the aim of improving the patients’ quality of life and preventing hospital admissions.

To arrange and interpret appropriate investigations to ensure evidence-based treatment and management plans to be created for all patients.

To undertake advanced clinical assessment.

To provide specialist advice, education and support to patients, their carers and other healthcare professionals, promoting patient centred care.

To provide support and assess competencies for trainee Heart Failure Nurses

Experience

Essential criteria

  • At least 2 years experience as a Heart Failure Nurse Specialist in UK setting
  • Evidence of continued professional development related to cardiology
  • Knowledge of cardiac pharmacology

Desirable criteria

  • Experience as Heart Failure Nurse Specialist
  • Evidence of Leadership experience
  • Experience in audit
  • Qualifications and Training

    Essential criteria

  • Registered Nurse
  • Mentorship / teaching and assessment qualification
  • Non-Medical Prescribing Qualification or working towards
  • Desirable criteria

  • Completed BSH Nurse Competencies or equivalent.
  • Post Graduate education relating to Speciality.
  • Heart Failure module
  • Post graduate education in history taking and patient assessment.

    Essential criteria

  • Knowledge of Cardiac Heart Failure Nursing and service delivery requirements
  • Excellent communication skills : interpersonal, written and verbal.
