Job summary

This is an exciting new ANP role, working for the PCN and based at Kennet & Avon Medical Partnership in Marlborough and Pewsey. The role will be predominately focused on same day, medically urgent patients.

Main duties of the job

Advancednurse practitioners work autonomously within the practice using their enhancedclinical assessment and treatment skills, to provide first point of contact forpatients presenting with undifferentiated, undiagnosed problems relating tominor illness or injury, abdominal pains, chest pains and headaches. They are autonomous in making decisions based onassessment, diagnosis and interpretation of test results. They canindependently prescribe appropriate medication, evaluate or refer to otherspecialists if necessary. ANPs in this role will focus solely on medicallyurgent patients.

There will be a requirement to work a full day every Friday.

About us

This role will join a team of paramedics, nurse practitioner and duty GP's who are responsible for treating same day, medically urgent patients. The role will be based from either site and will also include some home visits.

Key duties and responsibilities

The following are the core responsibilities of the Advanced NursePractitioner. There may be, on occasion, a requirement to carry out othertasks; this will be dependent upon factors such as workload and staffinglevels :

1. Manage patients presenting with a range of acute andchronic medical conditions, providing subject matter expert advice

2. Implement and evaluate individual specialisedtreatment plans for chronic disease patients

3. Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects to the patientshealth

4. Provide advanced, specialist nursing care to patientsas required in accordance with clinical based evidence, NICE and the NSF

5. Undertake the collection of pathological specimens

6. Request pathology services as necessary

7. Process and interpret pathology and other test resultsas required

8. Maintain accurate clinical records in conjunction withextant legislation

9. Ensure SNOMED CT codes are used effectively

10. Chaperone patients where necessary

11. Prioritise health issues and intervene appropriately

12. Support the team in dealing with clinical emergencies

13. Recognise, assess and refer patients presenting withmental health needs

14. Be an extended and supplementary prescriber, adheringto extant guidance

15. Support patients in the use of their prescribedmedicines or over the counter medicines (within own scope of practice),reviewing annually as required

16. Contribute to practice targets (QOF, etc.), complyingwith local and regional guidance

17. Liaise with external services / agencies to ensure thepatient is supported appropriately (vulnerable patients, etc.)

18. Delegate clinical responsibilities appropriately(ensuring safe practice and that the task is within the scope of practice ofthe individual)

19. Support the clinical team with all safeguardingmatters in accordance with local and national policies

20. Understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately

21. Deliver opportunistic health promotion whereappropriate

Administrative requirements

2. be aware of data protection(GDPR) and confidentiality issues particularly within a PCN

3. use technology andappropriate software as an aid to management in planning, implementation andmonitoring of care, presenting and communicating information

4. review and process datausing accurate read codes in order to ensure easy and accurate informationretrieval for monitoring and audit processes.

Supervision

The postholder will haveaccess to appropriate clinical supervision and an appropriate named individualin the PCN to provide general advice and support on a day-to-day basis.

  • Experience

    Essential

  • Experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans.
  • Essential clinical skills including phlebotomy, ECGs, urine analysis and swab taking.
  • Working knowledge of Microsoft and GP practice and prescribing data
  • monitoring systems.
  • Ability to write comprehensive clinical notes, implement and evaluate care plans.
  • Desirable

  • Mentorship or supervisory skills training.
  • Ability to undertake post- operative care.
  • Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN.

  • Qualifications

    Essential

  • Registered Nurse with Nursing and Midwifery Council
  • Masters degree required for qualification post December 2020 refer to RCN Credentialing for Advanced Level of Nursing Practice.
  • Post graduate diploma or degree for Advanced Practice Qualification up to December 2020
  • Qualified Independent Nurse Prescriber on the NMC register
  • Meets NMC revalidation requirements in accordance with the NMC Revalidation booklet
