You will work as an integral member of the podiatry and diabetes foot protection teams providing highly specialist assessment, treatment and advice for the high risk and most complex cases. You will be based in community settings but will also be part of the wider multidisciplinary diabetes foot team at Ashford and St Peter's Hospital. You will participate in consultant led MDT Diabetes foot clinics and Podiatry / Diabetes Nurses led Hot Foot Clinics delivering high quality, innovative and responsive programmes of podiatric care. An integral part of this role will include managing the domiciliary wound caseload and as such it is essential to have access to your own transport.
You will have experience of managing high risk diabetes patients, preferably in an acute MDT setting as well as possessing post graduate studies / qualifications relevant to this post. You will require excellent communication / people skills and have the ability to maintain effective working relationships with the wider MDT, GP's and other health care professionals.
Good IT skills are essential as the role requires working across a range of clinical systems. Although not essential it would be desirable to be a non-medical prescriber.
Main duties of the job
Provide highly specialist diagnosticassessment, treatment and advice for high risk, ulcerated and the most complexcases.
To be responsible for managing the domiciliary wound caseload liaising with the admin team, community nurses and TVN's where required.
Be responsible for the day to day running of podiatry clinics in the community and in hospital sites, cascading information to the whole team, triaging referrals and requesting investigations.
Be a part of the wider multidisciplinary diabetes foot team (MDT and Hot foot Clinic) at Ashford and St Peter's Hospital providing podiatry care for both inpatient and outpatient services.
To be responsible for maintaining patientpathways between acute and community care.
To lead and implement a regular reviewof outcome measures across the service to ensure the service provided iseffective..
To be involved in the operational andstrategic development of the service, embracing developments in practice andevidence based medicine.
To act as a source of expertise and undertaketraining of podiatry students, help colleagues maintain and develop theirclinical skills in wound care, and educate other health care professionals onthe role of podiatric wound care.
To participate in podiatry service wideCPD activities, clinical audits, service pilots / projects and other researchactivities to support the development of evidence based practice.

Clinical
Towork as an integral member of the podiatry team to deliver high quality,innovative and responsive programmes of podiatric care to patients identifiedas high risk.
Toprovide early intervention whenever possible, to maintain mobility andindependence, and prevent disability.
Usecomplex assessments to diagnose and develop highly specialised treatment plansand provide effective podiatric treatments, using a problem-solving approachbased on clinical need and evidence based practice. This is predominantly withpatients with complex medical conditions such as diabetes.
Tobe responsible for a complex and specialised caseload involving theimplementation and co-ordination of highly specialised treatment regimesincluding advanced assessment, wound treatment and monitoring, as well asconsidering alternative management plans.
Toevaluate wound care interventions using outcome measures.
Tocommit to the philosophy of care with an emphasis on patient empowerment andindependence by providing education and information to help patients, whereappropriate, manage their own foot needs, including footwear advice.
To work as part of a multi-disciplinary teams with Consultants, GPs and AHPs, leading the podiatry input and acting as a source of advice to other health care professionals and patients.
Tobe highly proficient in comprehensive neurological and vascular assessment,wound debridement, advanced pathology, and gait analysis.
To undertake diagnostic procedures effectively usingappropriate equipment Doppler ultrasound, competently interpreting,recording, acting on and reporting the results.
To provide services at a variety of sites including patient's homes, clinics, health centres and hospitals, often working in challenging conditions and frequently in isolation.
Toobtain informed consent prior to treatment plans commencing.
Toensure all new patients are appropriately assessed and existing patients areregularly reviewed.
Toundertake specialist wound care treatment for the most medically complex ofpatients.
To be able to lead colleagues in the event of a clinical emergency during clinical sessions myocardial infarction, faints or hypoglycaemic attack.
To maintain clear, comprehensive and accurate clinical records. To update patient's medical history, evaluate progression and develop treatment plans as required.
Toensure personal and best practice when exposed to unpleasant working conditionsand maintain the highest infection control standards in challengingenvironments.
Toteach and mentor students, newly qualified podiatrists and podiatry assistantsin undertaking their relevant procedures.

Understanding pathology and prognosis of different medicalconditions, to ensure that patients are fully aware of their foot risk status.Provide empathy and support to patients as required.
Tocommunicate patient related information effectively including future managementneeds with Consultants, GPs, Allied Health Professionals, Tissue ViabilityNurses, District Nurses, Practice Nurses, Multi Disciplinary Teams, HealthVisitors, Carers, Social Services and peers to ensure co-ordinated delivery ofcare.
Towork with colleagues in neighbouring organisations to share and developclinical guidelines and best practice.
Toprovide health promotional activities with individuals and groups, foot healtheducation sessions as well as individually tailored education duringtreatments.
Activelyinvolved in RCA of major lower limb amputation to improve service deliveryacross all the involved professionals and organisations.
