Are you a Newly Qualified, or an experienced Social Worker, registered with Social Work England, looking for your next challenge? Then Cheshire West and Chester Council could be the perfect place for you. We are seeking six new social workers to join our Hospital Discharge team, either on a permanent or temporary basis. The positions are based council wide and if successful you will be working at both hospitals and council offices.
These roles cover any Cheshire West resident needing discharge support to leave hospital. However, we work with our team to consider preferred locations to maximise their time and use of skills. You will work hand in hand with medics, therapy, nursing & reablement teams all focused with the same goals of completing personalised holistic assessment to support patients home safely and without delay. It is an opportunity to celebrate your social work value base and ensure that the voice of the individual and their carers are acknowledged and form the basis of any discharge plan.
