We are seeking a Registered Mental Health Nurse (Band 6) to join our clients multi-disciplinary Memory Assessment Service (MAS) team in Morecambe. This is a fantastic opportunity to work in a specialized role, supporting patients and their carers through high-quality assessment, planning, and care.Key Responsibilities :

• Serve as an independent specialist practitioner, ensuring effective assessment, planning, consultation, and formulation towards diagnosis within the Memory Assessment Service.
• Demonstrate specialist knowledge and understanding of strategies and interventions for dementia treatment.
• Proficiently refer patients to other agencies within the service or external services as necessary.
• Confidently identify risk factors and take appropriate actions to mitigate them.
• Facilitate clear and effective communication between GPs, LCFT services, and non-statutory services to uphold the highest standards of patient care.
• Provide mentoring and clinical supervision to staff and trainees on placement.
• Actively promote Trust values within the MAS service, aligning with NMC and HPC codes of conduct.
