Location : Kent, Canterbury

Pay Rates : £18 - £28 Per Hour

Shift Patterns : Flexible

Kent and Medway NHS and Social Care Partnership Trust (KMPT) , rated Good in their most recent CQC report (with care and some services rated as Outstanding), provides mental health, learning disability and specialist services across the county. Nestled in the Garden of England with easy access to London, KMPT operates across 33 locations, where a mix of rural, coastal and urban housing is available within Kent.

What you can add :

• Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
• Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their well being.
• Signposting to appropriate care providers (including primary and tertiary care).
• Raising concerns to senior staff for support and management.
• Completing documentation and planning care for patients.
• Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
• Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.

The role will involve, but not be limited to you performing the above.
