FULL 5 DAYS / 37 hours per week – ASAP start

Must be a car owner / driver

Hybrid working : 2 / 3 days per week office based

Responsibilities :

• Support and assess people who are in discharge to assess services, including recuperation and rehabilitation beds, and reablement services at home.
• Complete carers assessments.
• Work in partnership with health colleagues to meet individual’s needs.
• Be creative and work with individual’s strengths and community assets to meet their needs.
• Carry out mental capacity assessments and complete best interests decisions.
• Ensure that people are safeguarded and any risks are appropriately minimised.
• Manage a case load and prioritise the competing demands and time pressures of ensuring there is continual flow through the discharge to assess services.
