Are you able to support vulnerable young people in the Barry area, who have housing related support needs, including those who require support to establish and maintain a tenancy and those who are homeless?
We are searching for a Youth Support Worker - for a permanent contract.
What will you do?
- Support people aged 16 - 25 across Barry who are affected by homelessness and struggling to find, secure or maintain appropriate accommodation
- Undertake assessments of need in accordance with the support assessment procedure and as directed by the Projects Manager.
- Undertake risk assessments and risk management plans, including safety planning in accordance with policies and procedures.
- Work within a multi-agency settings where possible (mainly internal projects), with travel across the Vale of Glamorgan.
- Liaise with colleagues to provide timely, appropriate support.
- Work within the Supporting People Gateway framework, liaising with staff as required for case management and allocation.
- Deliver support and assistance to each person in accordance with the ethos, values and policies of the charity.
