We are currently inviting applications for Community Staff Nurses to join our Community Nursing Teams in the City locality of Bradford. As a community nurse working within the planned care service, you will be providing high-quality care for people in their own homes, ranging from long-term conditions to complex and palliative care. You’ll be working on a rotational basis as part of a seven-day service, with the support of agile technology to help you deliver timely, high-quality care, with support from specialists across the Trust.

There are a variety of shift patterns available between the hours of 8am and 9pm, 7 days per week.

There are lots of development opportunities available to you as a community staff nurse to enable you to progress your career, offering promotion to senior nurses or further study to become a qualified specialist practitioner or advanced practitioner, along with other modes of study to enhance your skills in tissue viability or palliative care.

Applicants must be able to regularly travel extensively across the district to meet the needs and demands of the service. The job will be based in City.

Main duties of the job

To work as part of the district nursing team to ensure high-quality health care is delivered to patients in their own home and / or clinic setting. To work in collaboration with the wider Primary Health Care team, whilst participating in the assessment of health needs and delivery of care to the practice population. The service is delivered over a 7-day rota and postholders are expected to work flexibly in line with agreed operating hours to cover a 24-hour service.

    Experience

  • Understanding and / or experience of interagency working and adult protection.
  • Qualifications

  • RGN / RN / EN Current NMC registration
  • Teaching / Assessing in clinical practice
  • V100 prescribing for nursing or equivalent
  • Training

  • Able to demonstrate ongoing professional development including a willingness to undertake education and training to meet the needs of the service.
