Job summary

Are you an enthusiastic clinician with experience in assessing and diagnosing adults with ADHD? Do you hold a non-medical prescribing qualification? If so, we want to hear from you!

Partnering Health Limited provide a service to the Isle of Wight, receiving referrals for adults presenting with symptoms suggestive of Attention Deficit Hyperactivity Disorder. We are seeking a  Specialist Practitioner  to work  within our team to provide assessment, diagnosis, and pharmacological management of patients. This is a home-based, virtual role and you will be provided with all the kit and training needed.

PHL runs Integrated Urgent Care (formerly known as Hampshire Doctors on Call). This essential primary care service serves million patients across Hampshire, providing telephone clinical assessment, out of hours home visiting services and some face to face primary care appointments services in West Hampshire. Additionally the organisation is equipped to offer acute and primary care services as it benefits from a well-established organisational infrastructure and strong clinical leadership and expertise.

In early 2016 PHL took over operational and commercial responsibility for Guildhall Walk GP Practice in central Portsmouth. The service transition was smooth and PHL plans to work with further GP Practices as part of its primary care strategy.

Job responsibilities

PHLprovide a service to the Isle of Wight, receiving referrals for adultspresenting with symptoms suggestive of Attention Deficit HyperactivityDisorder. Patients engage with triage, assessment and, if receiving adiagnosis, may subsequently choose to proceed to pharmacological management oftheir condition.

Thisis a virtual role for a non-medical prescriber with experience of assessing andtreating adults with ADHD. You will be working alongside the Clinical Managerand other professionals to diagnose and manage patients within a virtual clinicsetting, including prescribing and titration of preparations in line with NICEguidance and clinical judgment.

Youwill have a sound understanding of the relevant NICE guidance (NG87) andquality standards (QS39) for ADHD in adults and have experience of using DIVAto diagnose ADHD.

Youmust be confident in considering other psychiatric co-morbidities whenundertaking assessment, ensuring that the presenting symptoms are attributableto ADHD. You should also have sound understanding of the therapeutic efficacy,side effects and interactions of any prescribed treatment.

Youwill work within a small team to provide excellent patient-centred care,participating in multidisciplinary meetings as required to review individualcases.

Asthis is a virtual role, you must be confident in conducting assessments byeither phone or video consultation. PHL will provide full training and supportto use the applications and equipment required.

Clinical

  • Work autonomously within the wider team, demonstratingaccountability, responsibility and professionalism when providing a qualityservice to our patients.
  • Undertake assessments for the purpose of ascertaining whethera patient meets the criteria for a diagnosis of ADHD, using both clinicalacumen and the DIVA tool.
  • Formulate a management plan for our patients, includingtreatment pathway, follow-up and review, discharge, and onward referral, asnecessary.
  • Actively promote self-care and health strategies to empowerthe patient, taking a holistic and person-centred approach.
  • Identify and appropriately referto the patients own GP for vital signs or ECG monitoring in line with NICEguidelines.
  • Identify when patients require the input ofexternal agencies and refer to primary, community, non-profit and secondarycare services.
  • Identify child and adult protection issues, social needs, and safeguardingconcerns and action these inline with protocols.
  • Record in a timely manner a true and accurate record of the information and advice given, and to generatean electronic record.
  • Undertakeassessments of risk at each consultation, implementing onward referral and riskmanagement plans.
  • Beresponsible for timeliness of consultations, within the booked appointmentsystem.
  • Alwaysensure the safety of patients, including provision of safety netting advice.
  • Providecover in the absence of the Clinical Manager, undertaking additionalresponsibilities as agreed, once confident and competent to do so.
  • Prescribing

    Prescribeappropriately for patients requiringinitial or repeat prescriptions, in line with NICE guidance and the BNF.

  • Conduct medication reviews as required,titrating dose and / or prescribing alternative treatment as dictated by patientneed.
  • Workindependently in reviewing medication, understanding boundaries of knowledgeand when to refer patients for further management with a relevant service.
  • Accuratelyidentify those patients at risk of abuse or drug diversion.

    Qualifications

    Essential

  • Must be registered with the Nursing and Midwifery Council, HCPC or other appropriate professional body.
  • Must hold an independent non-medical prescribing qualification and be confident with writing prescriptions for off-label and controlled drugs.
  • Proactively maintains knowledge and training of current developments in clinical practice.
  • Demonstrable experience of working with adults who have ADHD, including diagnosis and treatment.
  • Experience of independent patient consultation, working autonomously.
  • Sound clinical decision-making skills.
  • Experience of using EMIS or other electronic record system is desirable.
  • Experience of using the Electronic Prescribing System is desirable.
