We are seeking a dedicated professional to join Swindon’s Initial Contact Team, providing vital first-line support and safeguarding adults at risk.Role & Responsibilities :

• Triage and assess incoming referrals for adults requiring social care support.
• Conduct initial risk assessments to determine the level of intervention needed.
• Work collaboratively with multi-agency teams, including health professionals and community services.
• Provide advice, signposting, and crisis intervention where required.
• Maintain accurate case records and reports, ensuring compliance with statutory guidelines.Requirements :
• Qualified Social Worker (Degree or equivalent).
• Registered with Social Work England.
• Experience in adult safeguarding, assessments, and care planning.
• Strong knowledge of Care Act 2014, Mental Capacity Act, and relevant social care legislation.
