Job description

What's involved with this role : Interim Senior Hospital Discharge Social Worker Reference no : Leeds 0009 9153 / 1 Enhanced  DBS disclosure required Hybrid working

Take a key role in supporting the team alongside the team manager including supporting less experienced staff with complex cases.

Support with allocations and attending partner meetings in the absence of the team manager.

Responsible for own case load which will be based both in hospital and outside of hospital. Such work will include supporting safe and timely discharges from hospital and reviewing care in temporary placements and in people's homes.

Key responsibilities :

Undertake a wide range of complex assessments including Community Care and Best Interest Assessments and construct innovative and cost-effective solutions

Carry out Safeguarding investigations within multi-agency procedures

Carry out risk assessments and identify needs and options

Using a person-centred approach, develop and maintain effective relationships with service users, their families and carers enabling them to make informed choices

Promote independence and early intervention, helping resolve conflict where needed

Contribute to the enhanced screening of new referrals and existing service users

Work in courts with appropriate support to obtain and implement court orders

Contribute to the development of practice and services

Carry out specific pieces of work on cases that are allocated to other teams

Undertake the Practice Educator Level 2 role

Participate in the Approved Mental Health Practitioner (AMHP) rota where qualified Key requirements :

Appropriate social work professional qualification (BSc, BA, MA, DipSW, CQSW, or equivalent) with significant post-qualification experience

Registration with the Health & Care Professions Council (HCPC)

Considerable experience working in a statutory setting

Planning, managing, prioritising and evaluating work with service users

Providing a high level of effective written and verbal communication

Overcoming resistance and exercising assertiveness where appropriate

Negotiating, challenging and managing conflict

Supporting service plans

Maintaining accurate service user records, assessments and following policy and procedure
