Leaving Care Personal AdvisorDuration : Initially 3 monthsHours : 37 hours per weekRate : £28 umbrella an hour (£22 PAYE an hour)Working arrangement : Hybrid working, 3 days a week in the officeThe London Borough of Merton are looking for a Leaving Care Personal Advisor to join their Preparing for Adulthood plus team.Responsibilities : Provide practical help and direct support to young people and young adults using the service from age 16 and are in care, or have recently left care, or who are unaccompanied asylum seeking children, to become independent and inspire them to set ambitious goals for themselvesManage a caseload involving the assessment, planning, implementation and evaluation of appropriate actionUse agreed assessment frameworks in collaboration with individual to robustly assess their needs and to develop corresponding tailored intervention Pathway Plans setting out clear goals and who is going to provide assistanceIdentify accommodation needs and assist young people insecuring appropraite accommodation optionsProactively work as an advocate for young people, navigate complex and contentious situations securing the collaboration of other agencies and individualsRequirements : Previous experience as a leaving care personal advisor or similarRelevant qualification in Connexions, Youth Work, Careers, Social Services, Teaching, Health, Youth Offending / Probation ServicesExperience working with young people, including those at a risk in a variety of settings
