Job summary

The Advanced Nurse Practitioner(ANP) at The Chowdhury Practice is key in delivering high-quality,patient-centred care. As a highly skilled and experienced healthcareprofessional, the ANP has demonstrated advanced clinical competency andexpertise in diagnosing, treating, and managing a broad range of healthconditions. Operating autonomously, the ANP assesses patients, makesindependent clinical decisions, and initiates appropriate interventions,including prescribing medications and ordering diagnostic tests. Our ANPs Practicework closely with patients, developing relationships built on trust andcollaboration. By offering expert care without the need for GP referral, theANP ensures that patients receive timely treatment, reducing delays andenhancing overall healthcare access. Additionally, the ANP evaluates treatmentoutcomes and refers patients to specialists or other healthcare services whennecessary, ensuring comprehensive care. This role offers a unique opportunityto provide holistic, patient-focused care, empowering patients to achieve andmaintain optimal health within the primary care setting.

Main duties of the job

Manage acute and chronic medicalconditions, creating and evaluating treatment plans.

Provide advanced nursing care,wound care, and specialist clinics ( reviews).

Request and interpret pathologytests, and maintain accurate clinical records.

Lead chronic disease management,including maintaining disease registers.

Prescribe medications, deliverimmunisation programmes, and manage mental health needs.

Participate in clinical meetingsand respond to clinical emergencies.

Home visits where needed forpatients who are housebound

Here at The Chowdhury Practice we treat all patients and their families with respect, we have currently 6600 raw list size. Our mission is to provide the best high quality care for each patient. We are fully embrace the diverse and unique population we have. We have 2 GP partners, 2 salaried GP`s, 2 locum GPs, 2 ANPs, 2 Practice Nurses, 1 HCA, 1 HSCW, 2 care cordinators and a team of fantatic admin / reception staff. We also have a number of ARRS memebers of the team.

We are not a training practice currently, we do fully encourage PCN working and training for all staff.

We hold a zero tolerance policy on abuse to any staff. Shouting, foul language or aggressive behaviour is not acceptable and can result in us asking you to leave the practice.

Job responsibilities

The following are expectationsfor the role of Advanced Nurse Practitioner. There may be, on occasion, arequirement to carry out other tasks; this will be dependent upon factors suchas workload and staffing levels : these will be discussed beforehand.

Manage patients presenting witha range of acute and chronic medical conditions, providing subject matterexpert advice

Implement and evaluateindividual specialised treatment plans for chronic disease patients

Identify, manage and supportpatients at risk of developing long-term conditions, preventing adverse effectsto the patients health

Provide advanced, specialistnursing care to patients as required in accordance with clinical basedevidence, NICE and the NSF

Request pathology services whererequired

Process and interpret pathologyand other test results as required

Provide chronic disease clinics,delivering patient care as necessary, referring patients tosecondary / specialist care as required

Maintain accurate clinicalrecords

Ensure SNOMED CT codes are usedeffectively and correctly

Maintain chronic diseaseregisters

Chaperone patients wherenecessary

Prioritise health issues andintervene appropriately

Support the team in dealing withclinical emergencies

Recognise, assess and referpatients presenting with mental health needs

Implement vaccination programmesfor adults and children

Support patients in the use oftheir prescribed medicines or over the counter medicines (within own scope ofpractice), reviewing annually as required

Contribute to practice targets(QOF, etc.), complying with local and regional guidance

Liaise with externalservices / agencies to ensure the patient is supported appropriately (vulnerablepatients, etc.)

Delegate clinicalresponsibilities appropriately (ensuring safe practice and that the task iswithin the scope of practice of the individual)

Support the clinical team withall safeguarding matters in accordance with local and national policies

Understand practice and localpolicies for substance abuse and addictive behaviour, referring patientsappropriately

Deliver opportunistic healthpromotion where appropriate

Provide supervision and supportto other members of staff such as Healthcare Professionals, Practice Nurses,Nurse Associates and Nurse Practitioners

Attend clinical, MDT andpractice meetings

Home visits to houseboundpatients

Person Specification

Qualifications

Essential

  • Registered Nurse with Nursing and Midwifery Council
  • Master's Degree required for qualification post.
  • Post Graduate diploma or degree for Advanced Practice Qualification.
  • Qualified Independent Nurse Prescriber on the NMC Register.
  • Meets NMC re-validation requirements in accordance with the NMC Re-validation booklet.

Desirable

  • Experience of working in Primary Care setting
